SS-31 (elamipretide) is a synthetic four-amino-acid peptide that stabilizes the inner mitochondrial membrane by binding cardiolipin. It is FDA-approved as FORZINITY for Barth syndrome patients weighing at least 30 kg to improve muscle strength, or available as research-use vials.

DoseFrequencyDurationRoute
Forzinity (FDA Label) - Patients ≥30 kg40 mgOnce dailyContinuousSubcutaneous
Forzinity (FDA Label) - Adults with severe renal impairment (eGFR <30 mL/min, not on dialysis)20 mgOnce dailyContinuousSubcutaneous
TAZPOWER (Trial Context) - Elamipretide arm40 mg SC dailyDaily12 weeks per period, with 4-week washoutSubcutaneous
TAZPOWER (Trial Context) - Open-label extension40 mg SC dailyDailyUp to 168 weeksSubcutaneous

Cycle: Forzinity label use is continuous, once daily, per prescriber. TAZPOWER trial arms included 12-week periods or up to 168 weeks in the open-label extension.

Benefits and research observations: Endpoints were measured at week 12 (no significant difference vs. placebo), week 36 (improvement from baseline), week 64-76 (improved 6MWT), and week 168 (improved 6MWT and knee extensor strength). Clinicians may monitor renal function, injection-site appearance, disease-specific endpoints, and hypersensitivity history.

Handling and storage

Store unreconstituted product according to its label and maintain the cold chain where applicable. After any preparation, follow only validated instructions for the specific product and diluent. Do not use if packaging is damaged or the solution shows particles or unexpected changes.
